High School
Important Registration Information
If you receive an error stating your child's STUDENT ID is incorrect, that means that your child does not have a current sports physical on file with the school.
All participants in High School athletics at Farmington High School must be fully registered prior to the student's participation in the first practice and/or tryout.
Before registration can be completed, a student must have:
- A current sports qualifying physical examination (SPQE) on file with the athletic office (the sports physical must be good through the entire sport season and must be dated after 6/15/2023 for the 2025-26 year);
- Completed the online registration through RevTrak(link above); and,
- Paid the athletic fees.
Sports physicals can be emailed to Sue Hurtgen (shurtgen@farmington.k12.mn.us), faxed to the High School (651-252-2520), or dropped off in the 2nd floor Main Office at Farmington High School.
Online payment must be made with a credit card. Families requesting financial assistance or who wish to pay by cash or check must do so in person in the Athletics/Activities Office in the second floor office at FHS. Student's will not be allowed to practice or try out until their registration is complete.
